Ruth's Promise Survey
Ruth's Promise is a one-day a week adult day program that provides a safe, therapeutic and enjoyable environment for adults who cannot stay at home alone. The program is offered on Tuesdays from 9:30 AM - 2:30 PM at a cost of $10 per hour. Financial assistance is available on a sliding scale basis for those who qualify.
While Ruth's Promise is housed and supported by Prince of Peace Lutheran Church, the program is open to the community and participants of all faiths and beliefs are welcome, encouraged, and respected.
This survey consists of 15 total questions. You may respond to fewer questions depending on your answers. In addition, you may skip questions that are not marked as required.
Your responses are recorded once you submit and cannot be changed. However, you may take this survey several times and each submission is counted as a new record.
